Below is a brief one-page flyer that provides an overview of the referral network and the available services and resources.

You can begin your referral by clicking on the link below or by visiting us in-person at the U.D.T.J. Community Center.

For all referrals, participants will be contacted one week after their referral to see if they are satisfied with their care or in need of additional services. If not, they will not be contacted again but the individual should feel free to contact us if they are in need of any further assistance.

We work hard to ensure that we respond within 1-2 business days to ensure a rapid delivery of services.
